Sign-off for each customer site shall not be given upon completion of each individual site where the customer has requested for service in various locations.
Customer billing will commence on activation of service, and successful testing of the link notwithstanding the absence of an executed Certificate of Completion.
By submitting this form, the customer agrees that the Suburban Standard Terms and Conditions are hereby incorporated into this Service Request Form, and shall constitute a valid and binding contract with Suburban in the absence of a signed Master Service Agreement.
